FOUNDATION LAB COACHING AGREEMENT
MFT: Erica Brown, iCare LAB
Service: Stress & Behavioral Coaching
1. Nature of Services
Coaching is a structured, non-clinical service focused on stress management, behavior change, and personal development.
Coaching is not psychotherapy, medical care, or mental health treatment. No diagnosis or clinical intervention is provided.
2. Coaching Options
Starter Experience
4 sessions for $550 (one-time payment)
Sessions must be used within 8 weeks
Monthly Membership
Weekly Coaching – $500/month
4 sessions per month
Bi-Weekly Coaching – $325/month
2 sessions per month
Membership renews monthly unless canceled.
3. Payment Terms
Payment is required prior to the first session
Starter packages are non-refundable after the first session
Membership payments are billed monthly
4. Scheduling & Expiration
Sessions are booked via the website calendar
Starter sessions expire after 8 weeks
Membership sessions must be used within the billing month
24-hour notice required for rescheduling
Missed sessions without notice may be forfeited
5. Communication Policy
Coaching includes limited between-session communication for brief check-ins only.
Clients may send up to:
5 messages per week (Starter & Weekly Membership)
3 messages per week (Bi-Weekly Membership)
The Coach will respond within 24 business hours.
Business Hours:
Coaching and communication are conducted during standard business hours:

Messages sent outside of business hours will be addressed on the next business day.
No contact outside of business hours unless previously discussed and explicitly approved in advance for a scheduled session.
Messaging is intended for brief check-ins only and is not a substitute for scheduled sessions.
Coaching services do not include crisis support, emergency response, or real-time communication.
Extensive communication will be charged $25 per 10 minute interactions.
6. Business Hours
Coaching hours: Tuesdays and Wednesdays 1pm - 4pm; 6pm-8pm.
Messages outside business hours answered next business day
7. Confidentiality & Privacy
All client information will be kept confidential
Services are not HIPAA-covered healthcare
Electronic communication may not be fully secure
8. Boundaries
No therapy, diagnosis, or clinical treatment
No emergency or crisis services
No referrals provided
Extensive communication will be charged $25 per 10 minute interactions.
9. Client Responsibility
Client is responsible for participation, decisions, and outcomes.
10. Cancellation (Membership)
Membership may be canceled with 7 days’ notice before next billing cycle
