FOUNDATION LAB AGREEMENT

MFT: Erica Brown, iCare LAB
Service: Wellness Coaching

1. Nature of Services

Foundation LAB is a structured, non-clinical service focused on stress management, behavior change, and personal development.

Coaching is not psychotherapy, medical care, or mental health treatment. No diagnosis or clinical intervention is provided.

2. Coaching Options

Starter Experience

  • 4 sessions for $550 (one-time payment)

  • Sessions must be used within 6 weeks

Monthly Membership - Automatically renews

Weekly Coaching – $500/month

  • 4 sessions per month - 60 minutes each

Bi-Weekly Coaching – $325/month

  • 2 sessions per month - 60 minutes each

Membership renews monthly unless canceled.

3. Payment Terms

  • Payment is required prior to the first session

  • Starter packages are non-refundable after the first session

  • Membership payments are billed monthly

4. Scheduling & Expiration

  • Sessions are booked via the website calendar

  • Starter sessions expire after 6 weeks

  • Membership sessions must be used within the billing month

  • 24-hour notice required for rescheduling

  • Missed sessions without notice may be forfeited

5. Communication Policy

  • Coaching includes limited between-session communication for brief check-ins only.

    • Clients may send up to:

      • 5 messages per week (Starter & Weekly Membership)

      • 3 messages per week (Bi-Weekly Membership)

      • The Coach will respond within 24 business hours.

  • Business Hours:

  • Coaching and communication are conducted during standard business hours:

    • Tuesdays - Thursday 11am - 5pm; Fridays 11am - 2pm. Special accommodations may be considered and the final decision is up to iCare LAB.

  • Messages sent outside of business hours will be addressed on the next business day.

  • No contact outside of business hours unless previously discussed and explicitly approved in advance for a scheduled session.

  • Messaging is intended for brief check-ins only and is not a substitute for scheduled sessions.

  • Coaching services do not include crisis support, emergency response, or real-time communication.

    • Extensive communication will be charged $25 per 10 minute interactions.

6. Business Hours

  • Coaching hours: Tuesdays and Wednesdays 1pm - 4pm; 6pm-8pm.

  • Messaging business hours: Tuesday - Friday 11am - 5pm

    • Messages outside business hours answered next business day

7. Confidentiality & Privacy

  • All client information will be kept confidential

  • Services are not HIPAA-covered healthcare however, information is never shared without the expressed consent of the client.

    • iCare LAB implements a “Safe to Share” (STS) process where the client creates a word or phrase indicating information may be divulged.

    • If the STS word or phrase is not provided verbally or in writing, information remains confidential. This includes spousal sharing.

  • Confidentiality cannot be guaranteed through electronic communication (text/email) as it may not be fully secure

8. Boundaries

  • No psychotherapy, diagnosis, or clinical treatment

  • No emergency or crisis services

  • No referrals provided

  • Session are only held during business hours. Violation will result in termination of remaining services

  • Extensive text or verbal communication will be charged $25 per 10 minute interactions.

9. Client Responsibility

  • Client agrees that he/she/they assumes full responsibility for participation, decisions, and outcomes.

10. Cancellation (Membership)

  • Membership may be canceled with 7 days’ notice before next billing cycle
